Release Agreement Instructions:


1) Select the Agreement Form you need below

2) Click  the link  the file opens in Microsoft Word  at the top of the window you see file
edit view etc. Click File  then  click Save As  then select your destination and hit ok
or click File then  click Print to print the document only.

3) If you do not have Microsoft Word on your Machine  you can download a viewer from Microsoft

4) If you saved the Document simply go to where you saved it open it and select File then Print from the
top Menu to print the Document Fill out the Document and  Mail to the supplied address
